Registration Information Page 2008

Four forms are needed to attend either camp.  They are:  Registration Form, Medical Release Form, Release and Indemnity Agreement.  The Jr Camp Afternoon Activities form is for Junior Camp ONLY.

These forms are offered in Adobe PDF format and MS Word format.  There is a link to download the free Adobe Acrobat Reader. 

Three methods.

1. Left click on the format and it should pull up either the Adobe Reader or Word.  You then can print.

2. Right click on the format you wish.  Select the 'Save Target As' option to download to your pc.
